Ohio provider selects ACO clinical integration tool

Mount Carmel Health Partners in Columbus, Ohio, has chosen accountable care organization MedVentive’s MedVentive Population Manager to support the group’s clinical integration initiative and help partner physicians and the Mount Carmel Health System improve patient care and clinical outcomes.

As part of its clinical integration program, Mount Carmel is implementing physician-directed quality improvement programs, focusing on improving patient care and clinical outcomes, the Waltham, Mass.-based MedVentive stated.

MedVentive Population Manager’s care management workflow tools and patient outreach tools focus on provider tasks and patient follow-up communications, the organization added.
