The unnamed woman had concealed the weapon, which the MRI machine triggered to fire during an exam. 

An FDA adverse event report suggests that although the patient involved in the accident was screened for metal prior to undergoing her MRI, she did not disclose the presence of a “butt plug." 

The incident happened at a SimonMed location in Arizona after staff failed to remove a waist shackle from the inmate.

The accident happened on February 23, and it has since been revealed that it occurred at Kaiser Permanente’s Redwood City Medical Center facility.

The new recall, which includes more than 155,000 implantable pulse generators, is due to ongoing issues with the devices being unable to exit MRI mode and return to normal use.
